Please bring the following items with you when you register your child:

Child's Birth Certificate
Child's Social Security Card
Child's Immunization Record
Child's Medical/Insurance Cards
2 Proofs of Income
Verification of Residency (Voter registration card, property tax statement or verification of physical residency such as a utility bill.

NOTE: Transportation to registration will be provided to you if needed. Please call the Many Preschool Center Office at 318-256-6143.

Head Start Selection Criteria
